Case Management Program

Definition

Case Management is a collaborative process which is both specific and flexible in assessing, planning, implementing, coordinating, monitoring and evaluating the options and services for the individual patient across the continuum of care.

Purpose

The purpose of Case Management is to improve the overall health and well being of the patient by promoting continuity of care, ensuring cost efficiency, identifying resources, implementing, monitoring and evaluating a plan of action, and continuously communicating with the parties involved. SHARP contracts with vendors to provide high quality, medically necessary services to the Case Management clients. SHARP's Case Management program is conducted in accordance with all legal mandates while supporting the patient's ethical and moral directives.

The goals of SHARP's Case Management program are:

To ensure high quality patient care in the most cost-effective way by coordinating human and material resources.

To improve the continuity of care.

To provide information and assistance to the patient and the physician.

To educate the patient and family to increase awareness of their disease process.

To maximize overall patient health and well-being.

Referrals

Patients can access the Case Management Program through referrals by their physician, a hospital, their employer, or other healthcare providers. Patients or their families can also contact SHARP directly to find out more about the Program.

Case Management services are available to all members and families whose employer has a Case Management contract with SHARP, regardless of age, sex, national origin, race or sexual orientation.
